in a fifth grade classroom a boy turns to face a sweet kind girl and tells her she is ugly and has a big forehead tears fill the eyes of this sweet kind girl as the others look on and say nothing words can hurt like a knife and leave scars

Ouch. That is so painful to witness, and then have to figure out how to address on the spot without making the girl hurt even more. Poor little girl. And poor boy too, who, for whatever reason, needed to lash out at his classmate.

Were you taught as a child, when another child said something hurtful, "sticks and stones may break my bones, but words will never hurt me"? Well, words do hurt, and it's always hard to know how to comfort both the child hurt, and the one doing the hurting. I like the ladder-like feel to how you wrote this incident.
